Additional Forms

The Voorhees High School Health Office provides the following forms to help families communicate important medical information and ensure that students receive appropriate care and support during the school day.

Parents/guardians should review and complete any forms that apply to their student. Certain forms may require completion and/or authorization by a physician or other healthcare provider. Please submit all required documentation to the Health Office and contact the school nurse with any questions regarding which forms are necessary.

Health & Medical Forms

  • Asthma Treatment Plan – For students with asthma who may require medication, treatment, or an individualized asthma plan while at school.
  • Authorization for Medication Form – Required when a student needs medication administered during the school day. Please use one form for each medication.
  • Diabetes Forms – Includes information regarding diabetic supplies, medical orders, insulin, blood glucose testing supplies, and required authorizations.
  • Allergic Reaction / Epinephrine Forms – For students with potentially serious or life-threatening allergies who may require epinephrine or other emergency treatment at school.
  • Long QT Syndrome Health Care Plan – An individualized school health care plan for students diagnosed with Long QT Syndrome.
  • Seizure Action Plan – Provides the Health Office and school staff with important information regarding a student’s seizures, emergency response procedures, medications, and special considerations.

Athletic Physical Forms

  • NJ Preparticipation Physical Evaluation (PPE) – Forms used as part of the physical evaluation process for student-athletes. Please note that the Medical Eligibility Form is the only portion that should be submitted to the school; the remaining physical evaluation information is maintained by the healthcare provider.
  • PPE Medical Eligibility Form – Documents a student-athlete’s medical eligibility for participation in athletics and should be submitted to the school for inclusion in the student’s health record.

Completed forms should be returned to the Health Office so that appropriate plans and procedures can be in place to support your student throughout the school day.
