Physical Exams are required for all new students to Voorhees High School. Please drop off or mail a physical exam to the Voorhees Health Office by August 21st. Do not fax, scan or email them.
Dear Parents/Guardians,
Welcome to Voorhees High School. On this page, please find information concerning your child’s entry into a new school and our health requirements and services. All necessary forms can be downloaded from the packet above or separately in the sidebar.
New Jersey Statute 18A:40.4, Administrative Code 6A:16-2.2 and North Hunterdon-Voorhees Board of Education Policy require a current physical examination for all ninth grade and transfer students. The Annual Athletic Preparticipation Physical Examination form must be completed by you and your doctor and returned to the health office before your child starts school. This paperwork should be completed whether your child plans to participate in athletics or not. We can accept any physical exam done within 365 days of the start of school. Please review your child’s immunization record with your physician at this time and update it as needed. After receipt of a current physical exam, we will be able to assist with paperwork for athletics, driver’s tests, working papers, and college and camp records. Please do not fax, email or scan physicals. Please hand deliver an original or send through US mail. Make a copy for your records!
A yearly physical exam will also be needed for upperclassmen participating in interscholastic and intramural sports, marching band and ski club. Please visit our web page concerning athletic participation.
New Jersey state law requires all children to have health insurance. Please see NJ Family Care for health insurance information. Please contact the nurses for dental, immunization or eye glass assistance if needed.
During the school day, medications may only be given to students by the school nurses. Please use the Permission to Dispense Prescription Medications form for those needed during the school day. Your physician must either complete the top portion or give prescription. Parents (not students) should bring the medication to school in the original container labeled by the pharmacy.
Insulin, asthma inhaler and epinephrine (Epipen) are the only medications that may be carried by a student and the required paperwork must be completed and submitted to the health office. The Over-the-counter Medication Administration Permission allows the school nurse to administer medications which have been pre-approved by our school physician and you. This may be completed on Aspen.
Please notify the health office and guidance department in writing of any changes in emergency contact information during the school year and update it on Aspen. We will supply emergency medical information to your child’s teachers, coaches, transportation coordinator, and bus driver for the following conditions: visual and hearing impairment, seizures /neurologic disorders, concussions, asthma, severe allergic reactions, diabetes mellitus, bleeding disorders, and significant heart conditions.
Students who experience an injury at school or during interscholastic sports should contact the health office promptly and complete an accident report. Parents/guardians will then be sent a supplemental insurance application. A note from your physician is needed for illness or injury which prohibits participation in physical education and athletics. Clearance by the physician will be required to resume participation.
Please let us know about any medical accommodations with which we can assist your child prior to the first day of school and as they arise throughout the year. During the summer, we have limited hours so please leave a message and we will get back to you as soon as possible.
