To obtain working papers please download the papers from the NJ Department of Labor and Workforce Development website and click on the A300 employment certification form.
Students should see the school nurse to get the doctor’s certification filled out. If you have a recent physical (within last two years), the health office can fill it out. If you do not have a recent physical, you will have to see their doctor to get it filled in.
For NORTH HUNTERDON STUDENTS ONLY please use this scanned working paper document. Once you have the entire document filled out scan that and send to the Counseling Secretary Vivian Prado

Voorhees Working Papers Instructions
Any student under 18 years of age must have working papers in order to obtain employment.
Please make sure that sections A, B and C are filled out completely before submitting the working papers. Section F needs to have the student signature. Part D, E and F are completed by either Voorhees or North. During the school year Part C can be completed by the school nurse if a physical exam was completed within the current year and is on file in the nurse’s office. During summertime you will need to see your normal physician to complete section C.
The following is needed if the student does not attend Voorhees or North:
- Section E needs to completed by the school the student attends.
- The minor needs to sign Section F before submitting the forms to be signed.
- Proof of age (birth certificate, passport, driver’s license) must be provided.
