To obtain working papers please download the papers from the NJ Department of Labor and Workforce Development website and click on the A300 employment certification form.
Any student under 18 years of age must have working papers in order to obtain employment.
Forms need to be brought to the Main Office at Voorhees (908- 638-6116) or the Guidance Office at North (908-713-4199). If you need the forms signed in the summer, please contact appropriate office to ensure someone is available to complete the form.
Please make sure that sections A, B and C are filled out completely before bringing the working papers into the Main Office at Voorhees or the Guidance office at North. Section F needs to have the student signature. Part D, E and F are completed by either Voorhees or North. During the school year Part C can be completed by the school nurse if a physical exam was completed within the current year and is on file in the nurse’s office. During summertime you will need to see your normal physician to complete section C.
The following is needed if the student does not attend Voorhees or North:
- Section E needs to completed by the school the student attends.
- The minor needs to sign Section F before bringing the forms to be signed.
- Proof of age (birth certificate, passport, driver’s license) must be provided.
