Transportation Waiver: 

Per Policy 3541.1: “Each school year, a parent/guardian of a student who is eligible for transportation services under the law may sign a written statement waiving the student’s right to those services. This written statement shall be in the form that is determined by the Department of Education. For families that waive their right to transportation services and maintain that waiver for the entire school year, the parents will receive reimbursement in the amount of $75.00 per student that did not utilize transportation services for the entire academic year.” 

To find this form, please follow the steps below:

3. Click on the student name
4. Click Options
5. District Online Forms
6. NHV Transportation Waiver
7. Click Next 
8. Fill Out the Form & Submit Online

Transportation Waivers will be submitted for reimbursement at the end of the school year.
