Dear Voorhees Parents\/Guardians:

Aspen, the North Hunterdon-Voorhees district’s student information system, is now open to accept required annual health forms<\/strong> for the 2021-2022 school year and registration forms for Fall sports. Parents can register their student for Fall sports from July 1<\/strong> through August 6<\/strong>.\u00a0 All forms must be filled out in Aspen, with the exception of the physical form. Physical forms are required for sports and intramural registration and for any student new to the high school district.

The\u00a0original and signed\u00a0physical form must be dropped off or mailed to the health office (keep a copy for your records). The only exception is freshmen parents can turn in a photocopy as their child’s\u00a0previous school may have the original physical (all incoming freshmen and new students\u00a0must turn in a valid physical by August 1).\u00a0For a blank physical form, please visit the health office website – https:\/\/www.nhvweb.net\/vhs\/healthoffice\/<\/a>.<\/p>\n

Freshmen parents<\/em> – if you did not receive the automated email to set up your parent Aspen account, please contact our database manager, Sherri DiMarcello, at sdimarcello@nhvweb.net<\/a> and she will resend the information to you.

All Parents – Annual Health Forms<\/strong>:<\/p>\n

Log into your parent Aspen account and click on Page Directory on left hand side.\u00a0 Click on Health Forms<\/u> to fill out all required health forms for the new school year.\u00a0If you filled out health forms before July 1st<\/sup>, they are no longer valid and you will have to fill them out again for the 2021-2022 school year.\u00a0\u00a0

Fall Sports:<\/strong><\/p>\n

The Fall sports offered at Voorhees High School are listed below with names of the head coaches. To register your student for Fall sports, log into your parent Aspen account and click on Page Directory on left hand side. Click on Voorhees Sport<\/u> to fill out the required sports registration forms.\u00a0 You will select the sport of choice in the Consent for Participation Form. You can run a checklist report at the bottom of the Voorhees Sport section to see if you are missing any required forms.\u00a0

ImPact baseline test\u00a0for concussions is required and the test is valid for two years. Registered students will receive an email from the athletic trainer on how to complete ImPact testing at home. Only students who never had an ImPact test (i.e. incoming freshmen or new students) or whose test has expired will need to complete testing (trainer already reached out to juniors with expired tests). (Freshmen athletes \u2013 see information below about accessing your school email account; login information is posted in parent\u2019s Aspen account. Freshmen will receive Aspen and school email account information also at Freshman Orientation on August 25 when they get their Chromebook.<\/em>)

A valid physical form must be on file, which means it should be dated after August 9, 2020 as a valid physical is one that has not expired within 365 days of the start of sports.\u00a0Please click here for practice start dates and locations<\/a>. The school doctor must review all physical forms so please turn in the physical form as soon as possible so your child is not delayed from practicing on the first day.

Athlete\u2019s Responsibility for Sports Registration:<\/strong>

There is one form that student-athletes must fill out themselves in their own student Aspen account for sports registration. Athletes need to log into their student Aspen account, click on Page Directory on left hand side, then click on VH Student<\/u>, and complete the Sports Wellness Acknowledgement Form. In the form, athletes will review the Sudden Cardiac Arrest pamphlet from NJDOE and the Opioid Video from NJSIAA. Make sure they click Submit at bottom of form so the form goes through.

For freshmen athletes<\/em> \u2013 The technology department creates the student Aspen accounts and Google email accounts for all incoming freshmen and their login information will be listed in the parent\u2019s Aspen account after July 12th<\/sup><\/strong> when we rollover the system for new school year. Parents will find the information by logging into their parent Aspen account, clicking on Family top tab, clicking on student\u2019s name, then clicking on Technology tab along the top row. Username and Password for student to sign into their student Aspen account is listed under the Technology tab, see screenshot below. Password is the same for the student\u2019s email address \u2013 students can log in through Gmail to access school email account.<\/p>\n
