{"id":827,"date":"2023-08-09T13:36:53","date_gmt":"2023-08-09T17:36:53","guid":{"rendered":"https:\/\/www.nhvweb.net\/vhs\/healthoffice\/?page_id=827"},"modified":"2025-06-09T10:13:24","modified_gmt":"2025-06-09T14:13:24","slug":"frequently-asked-questions","status":"publish","type":"page","link":"https:\/\/www.nhvweb.net\/vhs\/healthoffice\/frequently-asked-questions\/","title":{"rendered":"Frequently Asked Questions"},"content":{"rendered":"\n<p>Below are some frequently asked questions that we receive in the Health Office.\u00a0<\/p>\n<h3 style=\"text-align: center;\"><b>WHEN TO STAY HOME<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Q: When should I keep my child at home?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A: The New Jersey Department of Health recommends that students with at least <\/span><b><i>two<\/i><\/b><span style=\"font-weight: 400;\"> of the following symptoms or combination of symptoms be isolated:\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fever<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Chills<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Rigors (Shivering)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Myalgia (Muscle Aches)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Headache<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Sore Throat<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Nausea<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Vomiting<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Diarrhea<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fatigue<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Congestion<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Runny Nose<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">The New Jersey Department of Health recommends that students with at least <\/span><b><i>one<\/i><\/b><span style=\"font-weight: 400;\"> of the following symptoms be isolated:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Cough<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Shortness of breath<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Difficulty breathing<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">New olfactory (smelling) disorder<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">New taste disorder<\/span><\/li>\n<\/ul>\n<h3 style=\"text-align: center;\"><b>COVID-19<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Q: What is your policy if my child tests positive for COVID-19?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A: Please notify the health office if your child tests positive for COVID-19. Your child may return \u20186\u2019 days after first testing positive, with the first day counting as day \u20180\u2019 (7 days in total).<\/span><\/p>\n<h3>\u00a0<\/h3>\n<h3 style=\"text-align: center;\"><b>PHYSICALS<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Q: Does my child need a physical?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A: According to the Board Policy, all <\/span><b>freshman <\/b><span style=\"font-weight: 400;\">and <\/span><b>transfer students<\/b><span style=\"font-weight: 400;\"> require a physical to be completed by your private physician within <\/span><i><span style=\"font-weight: 400;\">30 days<\/span><\/i><span style=\"font-weight: 400;\"> upon entry. Additionally, all student athletes require an annual physical. This is the <\/span><a href=\"https:\/\/www.nhvweb.net\/vhs\/healthoffice\/files\/2019\/07\/NHV-Physical-Exam-form.pdf\"><span style=\"font-weight: 400;\">state required physical form<\/span><\/a><span style=\"font-weight: 400;\">. The NHVSD district utilizes the same form for all students, non-athletes, and athletes alike. The first two pages are completed and signed by both parent and student, and then reviewed by the physician. The last two pages are to be completed by the physician, and <\/span><b>signed, stamped and dated<\/b><span style=\"font-weight: 400;\">. Incomplete forms will be returned to the student.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Q: How long is a physical valid?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A: According to the State of New Jersey, physicals are valid for 365 days (one year from the date of the actual exam).<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Q: Does my child require a new physical for each season?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A: No, but the health history questionnaire on Aspen must be completed each season. Physicals are valid one year from exam date. Should your child\u2019s physical expire during the season, it is <\/span><span style=\"font-weight: 400;\">strongly<\/span><span style=\"font-weight: 400;\"> suggested that an updated physical is submitted to the nurse\u2019s office. Once a current physical is on file, you do not need to have your physician complete the form for each season your child participates in.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Q: My doctor\u2019s office is busy right now. I can\u2019t get an appointment until\u2026 Can\u2019t the doctor just write a note? Can\u2019t my child just participate until then?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A: Unfortunately, no. Your physician must complete an annual physical and complete the NHVSD forms in full. This is a mandatory, universal state form.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Q: My doctor provides us with a computer printout. All the information is on this form. Is this sufficient?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A: The State of New Jersey mandates that the universal forms be utilized.\u00a0 Computer printouts will only be accepted as documentation of immunization records.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">Q: Can the Athletic Director, Athletic Trainer or the Coach clear my child for athletics?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A: No. In order to prevent any delay in your child\u2019s athletic participation, all documentation must be submitted to the School Nurse. The School Nurse and School Physician will in turn review documentation and update your child\u2019s eligibility on Aspen.<\/span><\/p>\n<p>\u00a0<\/p>\n<h3 style=\"text-align: center;\"><strong>OPIOID VIDEO<\/strong><\/h3>\n<p><strong>Q: <\/strong>Does our athlete have to complete the opioid video requirement for each season that they participate in or once year?\u00a0 And, where can that video be found?<\/p>\n<p>A: The opioid video must be viewed before each season that an athlete participates in.\u00a0 \u00a0There is one form that student-athletes must fill out themselves in\u00a0<strong>their\u00a0<\/strong>student\u00a0<strong>ASPEN\u00a0<\/strong>account for sports registration.\u00a0 Athletes need to log into their ASPEN account, click on Page directory on the left-hand side, then click on VH Student.\u00a0 Complete the Sports Wellness Acknowledgement Form.\u00a0 In the form athletes will review the Sudden Cardiac Arrest pamphlet from the NJDOE and the Opioid Video from NJSIAA.\u00a0 Please make sure you click\u00a0<strong>SUBMIT\u00a0<\/strong>at the bottom of the form to ensure that the form is completed.\u00a0<\/p>\n<p>\u00a0<\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<h3 style=\"text-align: center;\"><b>HEALTH HISTORY QUESTIONNAIRE\/PARENTAL PERMISSION FORM<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Q: My child participates in more than one sport during the school year. Can I just include all sports in one form?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A: The State requires that the form be completed, signed and dated, within 90 days of beginning <\/span><b>each<\/b><span style=\"font-weight: 400;\"> season.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">Q: My child is a freshman or transfer student and is not participating in athletics. Do I still need to complete the health history questionnaire?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A: Yes. This completed form provides the nurse with important baseline information pertinent to your child&#8217;s health management.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<h3 style=\"text-align: center;\"><b>SCREENINGS<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Q: What screenings are done by the Certified School Nurse?\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A: The State has provided guidelines as to which screenings target each age group. Screenings will be completed by the Certified School Nurse, unless your child has submitted physician documentation or parental exemption of the recommended screenings. Written referrals will be sent home, as warranted.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For your reference, the Certified School Nurse at Voorhees High School\u00a0 has provided the following screening timeline:\u00a0\u00a0<\/span><\/p>\n<p><b>9th grade<\/b><span style=\"font-weight: 400;\">: Recommended screenings include blood pressure and scoliosis. Complete physicals are required and performed by your private physicians. Screenings are rarely rendered by Certified School Nurse.<\/span><\/p>\n<p><b>10th grade<\/b><span style=\"font-weight: 400;\">:\u00a0 If a student has not been screened by their physician, the following screenings are rendered by the Certified School Nurse: Height, Weight, Blood Pressure and Vision. If students deny screening(s), written exemption is required.<\/span><\/p>\n<p><b>11th grade<\/b><span style=\"font-weight: 400;\">:\u00a0 If a student has not been screened by their physician, the following screenings are rendered by the Certified School Nurse: Height, Weight, Blood Pressure, Hearing, and Scoliosis. If students deny screening(s), written exemption is required.\u00a0<\/span><\/p>\n<p><b>12th grade<\/b><span style=\"font-weight: 400;\">:\u00a0 If a student has not been screened by their physician, the following screenings are rendered by the Certified School Nurse: Height, Weight and Blood Pressure. If students deny screening, written exemption is required. <\/span><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">Q: When are screenings done?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A: Screenings are usually done during directed study time. Passes may be sent to students to report to the nurse\u2019s office.<\/span><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<p>\u00a0<\/p>\n<h3 style=\"text-align: center;\"><b>PE EXCUSE \/ STUDY HALL ASSIGNMENT<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Q: What happens if my child cannot participate in PE?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A: Your child needs to see the School Nurse and will be assigned to a study hall.\u00a0<\/span><\/p>\n<p><b>NO PE = NO ATHLETICS FOR THAT TIMEFRAME<\/b><\/p>\n<p><span style=\"font-weight: 400;\">If your child has a <\/span><b>doctor\u2019s note<\/b><span style=\"font-weight: 400;\">, they will be excused from PE and assigned to a study hall.
