This form is available via the link above, or on the right hand side of the \u2018Health Office\u2019 VHS website.<\/strong><\/p>\n\n\n\n The<\/strong> PREPARTICIPATION PHYSICAL EXAMINATION FORM AND HISTORY FORM<\/strong><\/a> must be completed in full \u2013 including a<\/strong> VISION SCREENING<\/strong> (located on page three), BLOOD PRESSURE, PULSE, HEIGHT, and WEIGHT.<\/strong> If vision is not completed at time of physical, you must submit documentation of your child\u2019s comprehensive eye exam.
