{"id":1024,"date":"2019-07-30T09:00:13","date_gmt":"2019-07-30T13:00:13","guid":{"rendered":"http:\/\/www.nhvweb.net\/specialservices\/?page_id=1024"},"modified":"2022-08-09T10:05:33","modified_gmt":"2022-08-09T14:05:33","slug":"referral","status":"publish","type":"page","link":"https:\/\/www.nhvweb.net\/specialservices\/referral\/","title":{"rendered":"Referral"},"content":{"rendered":"<p><strong><em>\u00a0<\/em><\/strong><\/p>\n<p><strong><em>What is a referral? <\/em><\/strong><\/p>\n<p>A referral is a written request for an evaluation that is given to the school district when a child is suspected of having a disability and might need special education services.<\/p>\n<p><strong><em>\u00a0<\/em><\/strong><\/p>\n<p><strong><em>Who can make a referral? <\/em><\/strong><\/p>\n<p>\u25a0 Parents<\/p>\n<p>\u25a0 School personnel<\/p>\n<p>\u25a0 Agencies concerned with the welfare of students, including the New Jersey Department of Education<\/p>\n<p>If you believe that your child may have a disability, you may refer your child for an evaluation by submitting a written request to your school district.<\/p>\n<p><strong><em>\u00a0<\/em><\/strong><\/p>\n<p><strong><em>What happens when a referral is made? <\/em><\/strong><\/p>\n<p>Within 20 calendar days of receiving a referral, the school district must hold a meeting to decide whether an evaluation will be conducted. If an evaluation will be conducted, another decision is made about the types of testing and other procedures that will be used to determine if your child needs special education services. If an evaluation will not be conducted, recommendations may be made with respect to interventions or services to be provided the student in general education.<\/p>\n<p><strong><em>How to Make a Child Study Team Referral?<\/em><\/strong><\/p>\n<p>If a parent is requesting a comprehensive Child Study Team (CST) evaluation for a student, please be advised of the following procedure. (Please note, CST evaluation requests cannot be submitted via electronic means, e.g. e-mail, fax, and\/or telephone.)<\/p>\n<p>Your request must be in writing and include the following information:<\/p>\n<ul>\n<li>Student&#8217;s Name<\/li>\n<li>Date of Birth<\/li>\n<li>School<\/li>\n<li>Grade<\/li>\n<li>School Counselor<\/li>\n<\/ul>\n<p>Also include:<\/p>\n<ul>\n<li>Parent\/Legal Guardian&#8217;s Name<\/li>\n<li>Address<\/li>\n<li>Daytime Phone Number<\/li>\n<li>E-mail Address<\/li>\n<\/ul>\n<p>Include concerns which are leading you to make this request.<\/p>\n<p>Sign and date your request <strong><em>in ink<\/em><\/strong>.<\/p>\n<p>Mail or hand-deliver your request in a sealed envelope addressed to:<\/p>\n<p>For North Hunterdon High School:<br \/>\nMr. Robert Pellechio, Assistant Principal<br \/>\nNorth Hunterdon High School<br \/>\n1445 State Route 31<br \/>\nAnnandale, NJ 08801<\/p>\n<p>For Voorhees High School:<br \/>\nMrs. Jennifer Komoroski, Assistant Principal<br \/>\nVoorhees High School<br \/>\n256 County Road 513<br \/>\nGlen Gardner, NJ 08826<\/p>\n<p>Parent\/guardian will be contacted by a Child Study Team case manager within 20 calendar days after the receipt of the letter in order to schedule a meeting and discuss the concern.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>\u00a0 What is a referral?
