We would like to provide you with an overview of procedures and assist with any health care needs your child may have during their educational years at NHHS. Your child will not be permitted to receive medication from the nurse for a headache, cramps etc. if you do not complete this form in Aspen.<\/li>\n<li><strong>Emergency Medical\/Contact Information Form<\/strong>-Please complete in Aspen and list all contacts, be sure to update as needed. Answer \u201cyes\u201d or \u201cno\u201d to inhaler, Epinephrine and diabetes. Enter any allergies. This form is very important.<strong style=\"font-size: revert; color: initial;\">\u00a0<\/strong><\/li>\n<\/ul>\n<p><strong>Freshmen and New Transfer Students<\/strong>: The North Hunterdon-Voorhees Board of Education requires a physical examination for all ninth grade and transfer students. The exam must be dated within one year of the date of entry into school. A current immunization record should be submitted with the physical exam. If you do not have health insurance, please contact us for alternate arrangements. We have also provided a link to NJ FamilyCare Health Insurance to assist you, if needed. <strong>9<\/strong><strong><sup>th<\/sup><\/strong><strong>\u00a0Grade Physicals for incoming Freshman are due in the Health Office by August 1<\/strong><strong>st<\/strong><\/p>\n<p><strong>Illness\/Injury<\/strong>: A certified school nurse will assist with illness, injuries, or health related issues. In the case of serious injury or illness, the parents\/guardians will be notified. If parents\/guardians cannot be reached, designated emergency contacts will be notified to transport the student for medical follow-up or home. If the need for an ambulance\/emergency care occurs, the parent\/guardian will be contacted and the student will be transported to Hunterdon Medical Center Emergency Room via rescue squad. Therefore, it is critical the\u00a0<strong>EMERGENCY CONTACT FORM<\/strong> be completed in Aspen each school year. Please remember to update this form in Aspen if emergency telephone numbers change during the school year.\u00a0 Please<strong>\u00a0keep your child home<\/strong>\u00a0if they have had a fever of 100.3 or above within 24 hours, vomiting\/diarrhea, unexplained rash, or weepy red eyes. Students must be 24hrs free from fever\/vomiting\/diarrhea without medication.\u00a0 Please use good judgement before sending your student to school.<\/p>\n<p><strong>Medical Notes:<\/strong> All letters from medical providers,<strong>\u00a0other than attendance notes<\/strong>, should be submitted to health office. Trainers, coaches, guidance counselors, and teachers will be notified accordingly and as appropriate. Please be sure to submit a return to activities letter when your child\u2019s condition resolves so it does not hinder future eligibility for athletics, physical education, field trips and\/or activities. Please note that the health office can not excuse your child from PE class without a note from your child\u2019s provider.<\/p>\n<p><strong>Medications\/Medication Forms<\/strong>: NJ and NHHS policy prohibits students from carrying any prescription or over the counter medication with the exception of inhalers for asthma, epinephrine auto-Injectors for severe allergic reactions, and Insulin\/glucagon for diabetes.\u00a0Medications required during the school day can be stored in the health office and administered by the school nurse with the submission of the NHHS Medication Administration Form. All medication must be delivered to the health office by the parent\/guardian in an original pharmacy container. Epinephrine, inhaler, and diabetic forms must also be submitted in order for your child to participate in athletics, field trips and other school sponsored activities. It is the student\u2019s responsibility to carry their emergent medications to all practices and games. <strong>Please note that all medication orders expire on the last day of each school year, therefore, all medication not picked up by a parent or guardian after the last day of school will be discarded.<\/strong><\/p>\n<p><strong>Screenings:<\/strong><strong>\u00a0<\/strong>All students will be screened for Height, Weight, Blood Pressure, Vision, Hearing, and Scoliosis as per NJ guidelines. If you wish to exclude your child from health screenings, please complete the <strong><em>Opt Out of Screening Form<\/em><\/strong> and return to the Health Office.<\/p>\n<p><strong>Sports\/Intramural Participation<\/strong>: Please visit the <strong><em>Ask the Medical Staff<\/em><\/strong> and <strong><em>NHHS Annual Health Forms and Fall Sports Registration <\/em><\/strong>link on the right side of this page for detailed instructions regarding sport\/intramural registration. Please run a requisite checklist in Aspen for the most up to date information regarding your child\u2019s clearance which includes the date on record of your child\u2019s most recently submitted PPE Medical Eligibility Form. In order to avoid delays in clearance please allow for advanced submission of the PPE Medical Eligibility Form as it must be reviewed by our school doctor which can take up to one week.<\/p>\n<p><strong>School Accident Insurance<\/strong>: The NHVRHS Board of Education has purchased insurance coverage that acts as a supplemental\/secondary insurance to assist all students with accidental injury. Coverage is provided for school sponsored and supervised activities. Although this coverage is very broad, there are restrictions, limitations and exclusions in this policy. In many situations, medical bills may not be covered in full. The NHHS Accident Report will need to be completed by the student in the Health Office for any school related injury. Documentation from your child\u2019s provider of medical care\/visit must also be submitted to the health office.
