Dear Parents\/Guardians:<\/span><\/p>\n

Please use Aspen to submit required forms to participate in athletics.<\/span><\/p>\n

If your child is interested in participating in a sport, please login<\/a><\/strong><\/span> to your parent Aspen account.<\/span><\/p>\n

Click HERE<\/a> <\/strong><\/span>and follow the instructions\u00a0to submit the Health History Update Form and the Consent for Athletics Participation Form online.\u00a0 In addition to these required forms, Aspen can run a report that will show if you are missing any other documentation necessary for sports participation.\u00a0<\/span><\/p>\n

STUDENT INSTRUCTIONS:<\/span><\/strong><\/span><\/p>\n

Dear Students:<\/span><\/p>\n

If you are interested in participating in a sport, submission of required forms must be done through Aspen.\u00a0 Your parent\/guardian needs to login<\/a><\/span><\/strong> to his\/her Aspen account.\u00a0<\/span><\/p>\n

*New requirement for student-athletes beginning winter season 2018-2019 – you must log into your student Aspen account, click on Page Directory, click on Nh or Vh Student, and complete the Sports Wellness Acknowledgement Form. This form includes the NJSIAA Opioid Video and the NJDOE Sudden Cardiac Death pamphlet. Review both, click on Yes for each one and submit the form.<\/p>\n

Student can run a report that will show whether all of the required athletics documentation has been submitted for you to participate.\u00a0 Please see the attached directions on how to run the checklist report.<\/span><\/p>\n
