North Hunterdon-Voorhees Regional High School District (NHVRHSD) is requesting proposals (RFP) for School Physician Services.<\/p>\n\n\n\n

Proposals must be submitted in a sealed envelope and delivered or mailed to the Office of
the School Business Administrator\/Board Secretary of NHVRHSD, 1445 State Route 31,
Annandale, NJ 08801 and received on or before Thursday, June 5, 2025 at 10:30 A.M.<\/p>\n\n\n\n

Requests For Proposal: RFP 2025-03- School Physician Services<\/a><\/p>\n\n\n\n

ADDENDUM I<\/strong>: The RFP is being amended to include within the Scope of Services: <\/em><\/p>\n\n\n\n

\u25cf A School Physician will provide coverage for all home football games and sign off\/review sports physicals<\/em>.<\/p>\n\n\n\n

<\/p>\n\n\n\n

ADDENDUM II<\/strong>: The RFP is being amended to exclude from the Providers Checklist (page 4):
Required w\/Proposal<\/strong>
\u25cf Language\/reference listed with \u201cTechnical Specifications\u201d. Technical Specifications are still
required; omit the language \u201c(E. Required Information\/Documents)\u201d.
Required Prior to Issuance of a Contract within 7 Days<\/strong>
\u25cf Omit \u201cProof of Criminal History (Item D. Technical Specifications)\u201d<\/em><\/p>\n\n\n\n

The sealed packages must be delivered between the hours of 9:00 a.m. and 2:00 p.m., Monday through Friday, excluding legal and district holidays.  The envelope is to bear the following information:<\/p>\n\n\n\n

The Board of Education does not receive electronic (email) submissions at this time.<\/p>\n\n\n\n

Respondents are required to comply with the requirements of N.J.S.A. 10:5-31 and N.J.A.C.
17-27 et seq.<\/p>\n\n\n\n


The Board of Education reserves the right to reject any proposals, pursuant to N.J.S.A.
18A:18A-2(s), (t), (x), (y), N.J.S.A. 18A:18A-4(a-c), and N.J.S.A. 18A:18A-22, and to waive
minor informalities or non-material exceptions, that may be in the best interest of the Board.<\/p>\n\n\n\n
