MARCHING BAND MEMBERSHIP FORM
Please get this in as soon as possible, but no later than June 30th. Membership to marching band may not change (additions or deletions) past June 30th.
PHYSICAL EXAMINATION FORM
This form is due in to the school nurse by July 31st. Any student participating in an extra curricular activity must complete this form as per district policy. This form should be submitted to the nurse along with the parent permission form.
ASPEN REGISTRATION INFORMATION
Parents of band members must sign up for an account and fill out all required forms in Aspen: Parent Permission Form, Consent to Random Drug Testing, Steroid Testing, and the form regarding concussions. Please note: ASPEN Registration will only be open from July 1st through July 31st.
MARCHING BAND FEES
This form is due ON THE FIRST DAY OF BAND CAMP. Included on the form are various mandatory and optional fees for participation during the season. This form should be returned to Mr. Giannelli at on the first day of camp – do not turn in early, do not mail the form, please turn it in person as requested on the first day of camp.
