Sport physical exams and parent permission forms for the 2006 spring season are due to the health office by Friday, February 16th.

Practices may start Saturday, March 3rd.

Please make sure all forms are complete, especially the vision  portion. Incomplete forms and those turned in after the February 16th deadline will cause delays in participation. All new physical exams must be reviewed by the school medical examiner before participation. Please plan accordingly!
