Summer Information

Freshman and New Transfer Students: The North Hunterdon-Voorhees Board of Education requires a physical examination for all ninth grade and transfer students. The exam must be dated within one year of entry into school and needs to be documented on the NHHS Student Physical Form. A current immunization record should be submitted with the physical exam. Forms are available on this website or the Health Office.

If you do not have health insurance please contact us for alternate arrangements. We have also provided a link to NJ FamilyCare Health Insurance to assist you, if needed. 

9^th Grade Physical Forms are due in the Health Office by August 1^ST

Illness/Injury: A certified school nurse will assist with illness, injuries, or health related issues. In the case of serious injury or illness, the parents/guardians will be notified. If parents/guardians cannot be reached, designated emergency contacts will be notified to transport the student for medical follow-up or home. If the need for an ambulance/emergency care occurs, the parent/guardian will be contacted and the student will be transported to Hunterdon Medical Center Emergency Room via rescue squad. Therefore, it is critical that PAGE A 1 of the ANNUAL PHYSICAL EXAMINATION FORM be completed each school year. Contact us if emergency telephone numbers change during the school year. 

Medications: State law does not permit students to carry prescription or over the counter medication while at school except in the case of potentially life threatening illness (Inhalers for Asthma and Auto-Injector Epinephrine for severe allergic reactions).  If it is necessary for a student to receive medication during the school day, the school nurse will administer it.  State law requires a written prescription signed by your health care provider including diagnosis, medication dosage, route and time of administration, and written consent of the parent/ guardian. The medication is to be delivered to the health office by the parent in the original pharmacy container.  Forms are available in the health office and on this site.

Screenings:   All students will be screened for Height, Weight, and Blood Pressure. 
11^th grade students will be screened for vision and hearing.  
9^th and 11^th grade students will be screened for scoliosis if they do not submit a current physical.
A pupil may be exempt from the scoliosis exam if we receive written parental/guardian notification. Complete, sign, and return the Scoliosis Parent Form in the sidebar and return to the Health Office.
Screenings may be requested by teachers, parents, or students if there is a health concern.

A letter will be sent home for further medical follow-up on any screenings that fall outside of normal parameters as defined by NJ state code or as deemed necessary by the NHHS School Physician.

Reminder: The Health History Questionnaire (Part A 1-3) must be dated within 60 days of the first practice date of the sport. All questions must be answered and all forms must be signed and dated.

The Physical Exam must be on the NHHS Annual Physical Examination Form (Part B 4-5), be dated within 365 days of the first practice date of the sport, and be completed in its entirety – this includes Height, Weight, Blood Pressure, Pulse, and VISION. Remember to bring your glasses or wear your contacts when you visit your health care provider.

Special Note:  It is the students responsibility to carry their prescribed medications for life threatening conditions (Inhaler; Epinephrine; Diabetic supplies, etc.) to all practices and games.

School Accident Insurance: The NHVRHS Board of Education has purchased insurance coverage to protect all students against accidental injury. Coverage is provided for virtually all school sponsored and supervised activities. Although this coverage is very broad, there are restrictions, limitations and exclusions in this policy. In many situations, medical bills may not be covered in full. The NHHS Accident Report will need to be completed by the student in the Health Office for any school related injury.
