RANDOM DRUG TESTING CONSENT FORM
This form is due by August 2nd.  Any student participating in an extra curricular activity must consent to random drug testing as per district policy.  This form should be returned to Mr. Giannelli.

PHYSICAL EXAMINATION FORM
This form is due in to the school nurse by July 19th.  Any student participating in an extra curricular activity  must complete this form as per district policy.  This form should be submitted to the nurse along with the parent permission form that you received in the mail  from the school nurse.

MARCHING BAND FEES
This form is due by August 2nd.  Included on the form are various mandatory and optional fees for participation during the season.  This form should be returned to Mr. Giannelli.

BAND CAMP CHECKLIST
This file includes a checklist of required and recommended items that you will need for the week of band camp.
