Required Athletic Forms

REQUIRED MEDICAL FORMS

Ask the Athletic Trainer

• Click the necessary button for the form needed. All forms are in Adobe Acrobat format (.PDF file). After completion the forms should be given to the Nurse's Office.
   

• All forms for athletics are required to be in by the due date. Fall August 1st, Winter November 15th, Spring February 15th annually.  

 

Anyone who missed May Impact Testing should test in August with their sport team. If you cannot make this date, you can attend another session.

ImPact Concussion tests will be offered on the following dates and times for freshman. All tests are in the media center and last about one hour.

 Spring Impact Testing:

• Wednesday Jan 18 - Girls Track
• Tuesday Jan 24 - Baseball
• Friday Feb 3 - Softball and Boys Tennis
• Tuesday Feb 7 - Girls Lacrosse and Boys Track
• Friday Feb 10 - Boys Lacrosse and Golf

All tests begin at 3 pm and last for approximately one hour. All tests are in the Media Center. If you cannot make your teams scheduled test please see Mrs. Korbul in the Athletic Training room.

 

 

DUE DATES ARE:

• October 25 and November 7th, 2011 for Winter Athletics and Intramurals (Band is not required to get a physical)

• August 1, 2011 for Fall Athletics and Intramurals (Band is not required to get a physical)

Please do not sign and date the parent permission form (A-1) before sixty days of the first day of practice.
