There is always some confusion about the athletic paperwork. Here are some tips to help make your experience more pleasant.
*Keep a copy of your child’s physical. This way you will be able to look up their physical date without calling the school.
*Make sure the physical form is COMPLETELY filled out. If your doctor did not put in your child’s vision then the form is incomplete and will be returned to you.
*Your child’s vision must be better than 20/40 or they will need to be cleared by an eye doctor.
*If you can, schedule your child’s physical for sometime during the summer. This way the physical is good for all three of our seasons.
*The Interscholastic Sport (Athletics) & Intramural Consent form only needs to be filled out ONCE. If you fill it out in the fall you are good for the whole school year.
*On the Health History questionnaire, if you list a prior injury or medical condition please put the date or year the event happened in. This helps us determine if further clearance is needed or not.
*On Part B page 4 the form has a statement that says the history prepared by the parent/student must be reviewed by the examining provider at the time of the physical exam and then the doctor needs to sign that the student is cleared for sports participation. This only needs to be done when the physical is done. Not for each season.
*If you complete the Interscholastic Sport (Athletics) & Intramural Consent Form then you do not need to complete the separate Student Activities\Parking Form.
*After each sport season you are required to complete a new page A1-A3. Please do not send photocopies with expired dates. This form cannot be dated more than 60 days prior to the first day of practice. For example a fall sport starting on August 15 cannot be dated before June 17. Winter sports of November 15 cannot be dated before September 17, and Spring sports cannot be dated before December 17. There is a chart posted on the athletic website with updated dates.

FREQUENTLY ASKED QUESTIONS:
My son/daughter will be a freshman trying out for the_______ team. What paper work does he/she need???
As an incoming freshman or a returning upperclassman, your child will need the following to participate in tryouts.
1. Take the Impact Concussion Test and have baseline on file.
2. The physical exam packet, Form B1-B4. You must have had a PHYSICAL EXAM within ONE YEAR OF THE START DATE for your sport. The physical must be signed by your physician and include the physical exam date. For example, your child had a physical on April 15th 2011. She is trying out for soccer on August 15, 2011. Her physical would be good until April 14th of 2012.
3. The three page Health History Questionnaire Form A1-A3, signed and DATED by the student and one parent BY THE DATE LISTED IN THE CHART. Forms signed BEFORE this date will be returned because the forms cannot have been signed more than 60 DAYS prior to the start of the season (state law not our rule). For example, the girls soccer team will start tryouts on August 15, 2011. Forms can be signed and handed in to the school after June 17th 2011.
4. If your child needs an inhaler or Epi-pen, they will need to fill out the Inhaler and Epi-pen forms and have them signed by your physician and parent. This will only need to be done once this year and will be kept on file in the nurse’s office. These forms should be dated for the 2011-2012 school year.
5. The Interscholastic Sport (Athletics) & Intramural Consent Form will give consent for the athlete to play sports and\or intramurals. It takes the place of individual forms for drug testing and sportsmanship. Signing this form says that you agree with and are allowing your child to be entered into the Random drug testing pool.

You should read the packet before signing and dating. There is no specific date for this form but it will need to be turned in prior to athletic clearance being given.
My child played a fall sport. What do I need to send in for winter team tryouts???
Your child will need to send in an updated Health History Questionnaire Form A1-A3 Signed within 60 days of the first day of tryouts. (See Chart) No other forms are needed if your child was cleared in the fall.
My child’s physical was done on August 20, 2010 and due to my insurance regulations I cannot get him another physical until August 23, 2011. Will my child be cleared for tryouts???
If you are having this problem, please email the trainers. We will try to help get your child cleared for practice and understand the complexities of insurance. Specifically, He would not be clear until the new physical comes in. So if you have a morning appointment he would be clear for the afternoon. Hand in all other paper work earlier. This will avoid the five day clearance for paperwork and get your child cleared as soon possible.
INTRAMURALS
If my child wants to participate in intramurals what paperwork needs to be filled out?
If your child is going to participate in an intramural sport such as weight room/weightlifting program, speed and agility program, basketball, floor hockey, pickle ball, Frisbee, project adventure, power lifting. They will need to fill out the Health History Questionnaire forms A1-A3, dated within 60 days of the first day of intramurals, have a current physical form B1-B4 on file, and have filled out the Interscholastic Sport (Athletics) & Intramural Consent Form.
What if my child is playing a varsity/sub-varsity sport and wants to participate in intramurals.
My child is on the track team but is also involved in the lifting program for football, what paperwork do it need???
You need all of the paperwork on file, the physical, Interscholastic Sport (Athletics) & Intramural Consent Form and the Health History Questionnaire forms dated appropriately.
On the front of page A1 there is a place that asks what sport they are participating in and another area right next to it that asks if they are participating in an intramural sport. You should fill out both of these areas and your child can be cleared for both sports and intramurals with one set of paperwork.
Please remember that as per state rules, the Health History Questionnaire forms must be done every season and signed within 60 days of the opening day of their activity.
My child is doing the after school weight training program and also wants to play floor hockey, what paperwork does he/she need?
They will need the Health History Questionnaire forms A1-3 filled out under the activities section. Please list both weight room and floor hockey. Make sure the date is not more than 60 days old. They would also need to have a current physical on file and the Interscholastic Sport (Athletics) & Intramural Consent Form on file also.
My son is going to play intramural floor hockey. Does he need to fill out the physical forms too???
YES. All intramural athletes need to fill out the Health History Questionnaire and Physical Packet. Form A1-3 and B1-B4. Specifically, fill out the box for intramurals on Part A. page 1. They also need to sign the fill out the Interscholastic Sport (Athletics) & Intramural Consent Form.
Also, if they are in the weight room in the fall and want to play floor hockey in the winter they would then need to fill out another Health History Questionnaire Form A1-A3 dated appropriately. (See Chart)
